OPWDD must determine your loved one has a qualifying developmental disability that began before age 22 and is expected to be lifelong.
The Front Door is OPWDD's intake. Once eligible, you'll attend orientation, meet with a Front Door specialist, and pick a Care Coordination Organization (CCO).
Your CCO assigns a Care Manager who builds and maintains your Life Plan, connects you to services, and coordinates with providers.
The Life Plan documents goals, services, providers, and crisis pathways. It's the master document — get every priority in there.
Once enrolled in the HCBS Waiver, you can access services like Community Habilitation, Respite, Day Hab, Supported Employment, and Residential Habilitation.
Self-Direction puts you in control of an annual budget you can use to hire staff, fund respite, recreation, and community supports.
OPWDD has real timelines, and you have real rights. Most families who push, in writing, get unstuck.
