Adult Transition
Aging out, adult services, and what to do at 14, 16, 18, and 21
If you wait until 18, you're already late. Here's the realistic NY timeline.
By age 14
- ✓ Apply for OPWDD eligibility if you haven't already.
- ✓ Transition goals belong in the IEP (NY requires by age 15).
- ✓ Talk to your Care Manager about adult day programs that exist in your region.
At age 16
- ✓ Vocational assessment in the IEP.
- ✓ Visit 2–3 adult providers.
- ✓ Begin guardianship vs supported decision-making conversation.
At age 18
- ✓ Apply for SSI in the birth month (income/asset rules change at 18).
- ✓ Apply for adult Medicaid.
- ✓ File guardianship paperwork (if pursuing).
- ✓ Confirm OPWDD waiver enrollment is active.
Year of 21 (or graduation)
- ✓ Day program seat secured.
- ✓ Residential pathway decided: family home, supported housing, or certified residence.
- ✓ Transportation, supports, and weekly schedule documented in the Life Plan.
