OPWDD

Step 1 — OPWDD Eligibility

OPWDD must determine your loved one has a qualifying developmental disability that began before age 22 and is expected to be lifelong.

Documents needed

  • Psychological evaluation with IQ testing
  • Adaptive behavior assessment (Vineland or similar)
  • Medical records confirming diagnosis
  • School records / IEPs
  • Proof of NY residency

How to prepare

  1. Gather every evaluation you've ever had — even old ones.
  2. If testing is missing, request it through school or Medicaid first.
  3. Call OPWDD's Front Door: 1-866-946-9733.

What to ask your team

  • Which qualifying diagnosis is being used?
  • What documentation do you still need?
  • How long until determination?

If you are denied or delayed

  • You have a right to appeal — request a Fair Hearing within 60 days.
  • Submit any new testing or evaluations with the appeal.
  • Get help from Parent to Parent of NYS or Disability Rights NY.
