Step 5 — HCBS Waiver Services
Once enrolled in the HCBS Waiver, you can access services like Community Habilitation, Respite, Day Hab, Supported Employment, and Residential Habilitation.
Documents needed
- ✓ Life Plan with services listed
- ✓ Medicaid eligibility
How to prepare
- Pick providers per service — you can mix and match.
- Build a backup list per service in case primary is full.
What to ask your team
- • Which waiver services are we currently authorized for?
- • What's the start date for each?
- • Who's the contact at each provider?
If you are denied or delayed
- → Document the denial. Request the specific reason in writing.
- → Appeal Medicaid-side denials through Fair Hearing.
