Step 3 — Care Coordination (CCO)

Your CCO assigns a Care Manager who builds and maintains your Life Plan, connects you to services, and coordinates with providers.

Documents needed

  • Eligibility letter
  • Recent evaluations
  • List of current services

How to prepare

  1. Interview Care Managers before accepting — you can request a new one.
  2. Come to the first meeting with your top goals and crisis areas.

What to ask your team

  • How often will we meet?
  • Will you put next steps in writing after every meeting?
  • Who's your supervisor if I can't reach you?

If you are denied or delayed

  • If a Care Manager is unresponsive, escalate to the supervisor in writing.
  • You can change Care Managers within your CCO — or switch CCOs.
