Documents needed

- ✓ Recent evaluations
- ✓ List of current services
How to prepare
- Interview Care Managers before accepting — you can request a new one.
- Come to the first meeting with your top goals and crisis areas.
What to ask your team
- • How often will we meet?
- • Will you put next steps in writing after every meeting?
- • Who's your supervisor if I can't reach you?
If you are denied or delayed
- → If a Care Manager is unresponsive, escalate to the supervisor in writing.
- → You can change Care Managers within your CCO — or switch CCOs.
