Step 4 — The Life Plan

The Life Plan documents goals, services, providers, and crisis pathways. It's the master document — get every priority in there.

Documents needed

  • Existing Life Plan (if any)
  • Provider contact info
  • Personal goals list

How to prepare

  1. Bring written goals — don't rely on memory.
  2. Insist that respite, housing, and crisis pathways are documented.
  3. Review the Life Plan before signing.

What to ask your team

  • Is residential a documented goal?
  • What's the crisis plan if our home becomes unsafe?
  • When is the next Life Plan review?

If you are denied or delayed

  • If a goal is left out, send a follow-up email documenting the request — it creates a paper trail.
