What to say when calling OPWDD

Use this when calling the OPWDD Front Door for the first time, or following up.

Script

Hi, my name is [your name]. I'm calling about [loved one's name], date of birth [DOB], who I believe is eligible for OPWDD services.

[Choose one:]
— We have not yet started the eligibility process. Can you tell me what documentation you need and how I can register for the next Front Door information session?
— We are eligible but have not yet been assigned a Care Manager. We were determined eligible on [date]. Can you tell me the status?
— We have a Care Manager but haven't heard from them in [X weeks]. Their name is [name], CCO is [CCO]. Can I get the supervisor's contact information?

Please send me confirmation in writing by email at [email] today.

Tips

• Always ask for the call reference number.
• Note the date, time, and name of who you spoke with.
• Send a follow-up email summarizing the call within 24 hours.
