What to say when a loved one needs housing

Use with your Care Manager when starting the residential pathway.

Hi [Care Manager],

I'd like to formally request that residential placement be added as a goal on [name]'s Life Plan, and that a Vacancy / Residential Request be submitted to OPWDD.

Our situation: [aging caregiver / unsafe home / readiness / etc.]

Please tell me:
1. Which providers in our region we should tour
2. What documents you need from us
3. When the Life Plan will reflect this goal

I'd like to start tours within 30 days.

Tips

• Tour 2–3 providers before committing.
• Stay in touch with each provider monthly.
• Keep applying to Section 8 in parallel.
